Take I-91 North-bound to Exit 21, "Route 372, Cromwell, CT".
At the end of the exit, you'll see a Sunoco gas station straight ahead; turn right onto Route 372 West.

You will go under I-91 North-bound and South-bound, and past the entrance to I-91 South-bound.
